NOTICE OF HEARING

TO ALL PERSONS WHO CLAIM TO BE THE SURVIVING SPOUSE, CHILDREN, BROTHERS, SISTERS, GRANDPARENTS, GRANDCHILDREN OR HEIRS OF GILBERT ROSS FAIRCHILD, DECEASED, OR WHO CLAIM TO HAVE AN INTEREST IN THE DEATH CLAIM BASED ON THE DEATH OF GILBERT ROSS FAIRCHILD:

Notice is hereby given that a hearing will be held on the 26th day of March, 2013, at 9:00 a.m. at the Oklahoma Workers’ Compensation Court, 210 Kerr Office Building, 440 South Houston, Second Floor, Tulsa, Oklahoma before Judge Brad Taylor.  The purpose of the hearing will be to determine whether Gilbert Ross Fairchild died as a result of an on the job injury while working for Jack Kissee Ford, Inc., on June 25, 2010, and to further determine those persons entitled to receive workers’ compensation death benefits under Title 85 of the Oklahoma Statutes as a result of the death of Gilbert Ross Fairchild.

You are hereby advised to appear at said court on said date and time in order to present your claim for benefits.  Should you fail to appear, you will be determined to have waived any benefits to which you might be entitled as well as to contest the court’s determination as to whether Gilbert Ross Fairchild died as a result of an on the job injury while working for Jack Kissee Ford on June 25, 2010.
